Section 1200-23-03-.08 - LOSS OF PERMIT DOCUMENT

(1) Any establishment or operator that loses, misplaces, or destroys a permit or license shall immediately apply for a duplicate, as soon as the fact becomes apparent. The fee for the duplicate shall be three dollars ($3.00). This fee shall accompany the application for such duplicate.
