Section 1200-06-01-.16 - REPLACEMENT LICENSE

A license holder whose "artistically designed" license or renewal certificate has been lost or destroyed may be issued a replacement document upon receipt in the Board's administrative office of a notarized written request. Such request shall be accompanied by the required fee, pursuant to Rule 1200-06-01.06.
