Section 1140-15-.04 - COLLABORATIVE PHARMACY PRACTICE AGREEMENTS

Within 30 days from the effective date of a collaborative pharmacy practice agreement, the prescribing pharmacist shall submit written attestation to the Board for the purpose of notifying the Board of the collaborative agreement.
