Section 0800-07-01-.03 - COMPLAINT(1) If any state or local governmental agency, officer, employee or entity has reason to believe that a person has violated the Act, the agency, officer, employee or entity shall file a signed, written complaint with the Department.(a) The complaint shall be on an approved form and shall contain the following: 1. Sufficient information to identify the person filing the complaint and the person allegedly violating the Act, including their names and addresses; and2. Detailed facts and circumstances that form the basis of the complaint, including relevant persons, dates, times, place(s), and copies of relevant documents.(2) The Department's Labor Standards Division shall receive all complaints from a state or local governmental agency, officer, employee or entity.
