Section 0770-06-05-.02 - APPLICATION PROCEDURES

(1) Grants shall be made on the basis of applications submitted to the Executive Director by the applicant. The application shall include at least:

(a) List of proposed activities;

(b) Amount requested;

(c) Amount and source of matching funds;

(d) Targeting elements (physical or economic);

(e) Method of notifying the public of the program;

(f) Criteria for choosing applicants;

(g) Procedure for selection of contractor, if applicable;

(h) Number of households to be assisted;

(i) Itemized administrative budget.
