Section 0260-05-.11 - RETIREMENT AND REACTIVATION OF CERTIFICATE
(1) A person who holds a current certificate and does not intended to serve as a "certified chiropractic therapy assistant" may apply to convert an active certificate to retire status. An individual who holds a retired certificate will not be required to pay the renewal fee.
(2) A person who holds an active certificate may apply for retired status in the following manner:
(a) Obtain an affidavit of retirement form, complete, and submit to the board's administrative office.
(b) Submit any documentation, which may be required to the board's administrative office.
(3) Certification holders whose certificate has been retired, may reenter active status by doing the following:
(a) Submit a written request for certification reactivation to the board's administrative office.
(b) Pay the certification renewal fees and state regulatory fees as provided in rule 0260-05-.06. If retirement reactivation is requested prior to the expiration of one year from the date of retirement, the board will require payment of the late renewal fee and past due certification renewal fees.
(4) Certification reactivation applications shall be treated as certification applications and review decisions shall be governed by rule 0260-05-.07.
