Section 67:63:03:04 - Changes requiring notification

A designated facility shall notify the department before the following changes for a determination on continued designation:

1) A change in the facility director;

2) A reduction in services provided by the facility; or

3) The impending closure of the facility.

A designated facility shall provide the department 30 days written notice of closure.
