Each eligible individual applying for HOPE waiver services must have an individual support plan that is signed by the individual or the individual's legal representative.
The individual support plan must include:
(1) The individual's assessed needs, including health and safety risk factors;(2) The individual's personal goals, to be achieved either by the provision of waiver services or through other means; and(3) The type, scope, cost, duration, and frequency of each HOPE waiver service. The department shall revise the individual support plan annually or as the individual's needs change.
