Section 67:16:26:03 - Responsibility for reporting of third-party liability - Applicants

As a condition of eligibility, an applicant must provide information about the existence of any third-party payment source. The information includes items such as the name of the policy holder, the policy holder's relationship to the applicant, the policy holder's social security number, the name and address of the insurance company, and the policy number.
