Growth hormones are covered when the use of the drug has received prior authorization from the department. Either the prescribing physician, other licensed practitioner, or the pharmacist must complete the prior authorization form which is available from the department. Authorization for payment is based on diagnosis and medical necessity. The department will respond to the request for prior authorization within one business day after receiving the completed form.
