The receipt of the superintendent of election for ballots and supplies shall be in the following form:

I, ____________________, Superintendent of Election in and for the voting precinct of _______________ in said County, do hereby certify that on the ______ day of ______________, 20____, at the hands of _______________ (Auditor, Sheriff), by ________________, his deputy, of said County, I received a sealed package said to contain official ballots and necessary supplies for the use of the voters of said precinct at the _____________ election to be held on Tuesday the ______ day of ______________, 20____.
