Section 47:03:04:02.01 - Renewal process

Any case management plan which has previously been certified in this state shall, by July 1 of each year, renew and file any changes to the annual registration for the case management plan. Failure to file the renewal will result in a lapse in the case management plan certification.

The fee for initial or annual registration of a case management plan is $250.
