The petitioner or the petitioner's agent seeking a small claims hearing shall file with the Division of Labor and Management a petition for small claims hearing form provided by the department. The petition must state the facts upon which the claim is based. If the department deems the petition unclear or insufficient, the department may refuse the petition, or order that a clearer or more sufficient claim be made.
