Section 46:30:07:05 - Assessment and service plan

The department, in consultation with the consumer, shall assess the consumer's capacity to live independently and the consumer's attendant services needs. The department and the consumer shall use the assessment to develop the consumer's service plan. The case manager, consumer's personal physician, and department must approve the plan before the department may reimburse for services.
