Section 20:86:04:06 - Other fees

Any person licensed in this state and who has the following requests shall pay the stated fee:

(1) For providing a transcript, $25;

(2) For a name change on a record of the license holder, $100;

(3) For issuance of a duplicate license, $100; and

(4) For endorsement to another state, territory, or foreign country, $150.
