Section 20:86:01:02 - Information required

Any person seeking licensure shall submit a notarized application and the following information:

(1) Name, address, social security number, telephone number, and date of birth of applicant;

(2) Documentation of certification as required in SDCL 36-38-7;

(3) Official transcripts showing compliance with educational requirements required in SDCL 36-38-7. Applicants are responsible for having official college transcripts sent from the college to the board;

(4) A statement of any criminal record;

(5) A statement regarding disciplinary proceedings or pending complaints as required in SDCL 36-38-7; and

(6) A nonrefundable initial fee, if applicable, and licensure fee as specified in § 20:85:01:04.
