Section 20:80:04:03 - Trainee recognition

A completed application for ACT or PST recognition must include:

(1) A completed application;

(2) The applicable fees;

(3) Verification from a high school or approved program recognized by the board demonstrating the educational requirements for recognition, as prescribed by this article;

(4) Verification of the certificate or license number and status of the certificate or license from the applicable licensing board in each state where the applicant has been certified or licensed to practice counseling, if applicable;

(5) A statement of criminal record, if applicable;

(6) A statement of any previous discipline, including denial, reprimand, revocation, or suspension of a previously held certificate or license; and

(7) A recent photograph.
