A course provider shall provide an individual certificate of attendance to each licensee upon completion of the prelicensing course under the following conditions. The certificate shall contain the licensee's name, the course title, the date, the location of the course, the number of approved credit hours, and the signature of the course provider or instructor. No certificate of attendance may be issued to a licensee who is absent for more than ten percent of the classroom hours.
