Section 20:51:29:07 - Registration application form-Fee

The application form for registration as a pharmacy technician must contain:

(1) The applicant's name, address, phone number, date of birth, gender, social security number, and email address;

(2) The applicant's work experience;

(3) Current and past places of employment; and

(4) A non-refundable fee.
