Section 20:51:27:02 - Application form

The application form for licensure of a nonresident pharmacy must include the information required by SDCL 36-11-19.3 and:

(1) Evidence of licensure in good standing in the nonresident pharmacy's home state;

(2) A description of any disciplinary action against the nonresident pharmacy or the non-resident pharmacy owner, in the home state or any other state within the last four years and the reason for the action;

(3) If the pharmacist-in-charge is not the sole owner or part owner of the merchandise and fixtures of the nonresident pharmacy, an affidavit as described in SDCL 36-11-34;

(4) A list of all other states in which the pharmacy is licensed; and

(5) A description of pharmacy services provided to patients located in South Dakota.
