Each applicant for licensure shall apply to the board on prescribed forms. Applicants shall attest that they have reviewed and agree to comply with South Dakota optometry law and ethics.
Applicants for licensure shall also submit a set of fingerprints on a standard card provided by the board for the purpose of obtaining a state and federal criminal background check pursuant to SDCL 36-7-12.2. The applicant must sign and submit a release form authorizing the release of the criminal history to the board.
