All applications for licenses and registrations to participate in racing shall be made to the commission on forms supplied by them.
An individual, partnership, or corporation applying for a license from the commission must provide his name and all names of the partnership or corporation, with any nicknames or aliases; a permanent address at which the applicant can receive official correspondence from the commission; a local address; home and business telephone numbers, age, date of birth, height, weight, color of hair, color of eyes, sex, marital status, citizenship, place of birth, social security number, and next of kin; previous racing license history; any rulings made against the person, partnership, or corporation by any racing jurisdiction; any arrest or criminal history; and any other information which the commission may consider necessary. The information must be provided on an application form provided by the commission. In addition, the commission may require the applicant to provide photographs or fingerprints, taken at a time and place and in a manner required by the commission. The application form shall include a statement that certified mail was sent to the permanent address of a licensee, as listed on the license application, whether delivered or returned to the commission for any reason, constitutes reasonable notice of licensing or disciplinary action by the commission.
