Fees generated by the following fee schedule shall be retained and expended by the Department to offset the cost of operation of the Vital Records System.

(1) Correction of certificate by affidavit | $ 15.00 |
(2) Amended certificate (adoption, legitimation court order, acknowledgment | paternity $ 15.00 |
(3) Delayed Registration of Birth | $ 15.00 |
