Subsection 61-125.901 - General. (II)A. The written informed consent between the patient and the facility shall include at least the following: 1. An explanation of the specific care, services, and/or equipment provided by the facility;2. An explanation of the conditions under which the patient may be discharged and the agreement terminated; and3. Documentation of the explanation of the patient's rights (see Section 1000) and the grievance procedure.B. The facility shall provide necessary items and assistance, if needed, for patients to maintain their personal cleanliness.C. The provision of care and services to patients shall be guided by the recognition of and respect for cultural differences to ensure reasonable accommodations for patients with regard to differences, such as, but not limited to, religious practice and dietary preferences.D. The facility shall make opportunities for participation in religious services available.
