If you still are interested in admission to (Name of Section 8) then you must notify the undersigned within 90 days of the date listed above. If you fail to notify us of your continued interest within that time, your name will be removed from the waiting list and you will no longer be considered for an apartment at (Name of Section 8). You should notify us in writing by using the attached form. Complete the form, detach it from this letter, and send it to this office by mail or in person within 90 days of the above date. You should keep a copy of the completed form for your own records. If you are no longer interested in admission to (Name of Section 8), you do not have to contact us. Your name will be automatically removed from the waiting list after 90 days from the above date.
