Section 11.266 - Preparation of license(a) The license is issued to the legal entity.(b) The license lists the name of the legal entity, the name and address of the center, type of services provided, maximum capacity, title and subchapter of applicable license regulations, the date the regulations were adopted, license number, effective and expiration dates of the license and restrictions, if applicable.(c) A provisional license also contains the word "provisional" and indicates whether it is the first or second provisional license issued.(d) An interim license also contains the word "interim."
