Section 33.334 - Application for permit(a) A dentist who desires to obtain a permit to administer general anesthesia, deep sedation, conscious sedation or nitrous oxide/oxygen analgesia, or a temporary permit, shall submit an application on a form provided by the Board, pay the permit fee prescribed in § 33.339 (relating to fees for issuance of permits) and meet the requirements for the permit applied for as prescribed in this subchapter.(b) Application forms may be obtained from the State Board of Dentistry, Post Office Box 2649, Harrisburg, Pennsylvania 17105-2649.The provisions of this § 33.334 adopted July 8, 1988, effective 7/9/1988, 18 Pa.B. 3045; amended May 13, 2005, effective 5/14/2005, 35 Pa.B. 2880.The provisions of this § 33.334 issued under sections 11.2 and 11.3 of The Dental Law (63 P.
