Section 1.3 - Amendments to birth certificatesThe following documentary proof shall be required of persons making application for an amendment of a birth certificate following a change in civil status:
(1) When the change involves a person whose birth is registered as illegitimate but who is legitimated by the subsequent marriage of his natural parents, a copy of the marriage certificate of the natural parents shall be submitted together with a joint affidavit of the husband and wife signifying that they are the natural parents of the child.(2) When the parentage of the child has been determined by a court of competent jurisdiction a certified copy of the court order shall be submitted.(3) When the child has been legally adopted a certified copy of the decree of adoption or an official notice from the court of the fact of adoption shall be submitted. The notice shall include all facts necessary to locate, identify and amend the original certificate of birth of the person adopted.(4) When the name of a person is changed by order of a court of competent jurisdiction a certified copy of the court order shall be submitted.
