Section 101.111 - Application forms(a) An application for a license to conduct a school shall be made on forms furnished by the Department.(b) An applicant for a school license shall certify, through completion of the self-authenticating document provided by the Department, its compliance with the act.(c) An application shall be accompanied by a sworn affidavit certifying the truth of the statements made in the application. An application may not be submitted to the Department by facsimile because of the required notary public certification.(d) The Department will provide its approval of applications either by mail or facsimile.
