Section 850-030-0035 - Licensure Fees; Certifications; Miscellaneous

(1) Licensure Fees for initial active License

(a) Licensure application fee; pro-rated schedule by date of application:

(A) January 1 through March 31, $400

(B) April 1 through June 30, $325

(C) July 1 through September 30, $250 and

(D) October 1 through December 31, $175.

(b) Annual Prescription Drug Monitoring Program (PDMP) fee $35

(c) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(2) Annual Renewal Fees for Active License:

(a) Licensure renewal $400

(b) Annual Prescription Drug Monitoring Program (PDMP) fee $35

(c) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(3) Annual Renewal Fees for an Inactive License

(a) Licensure renewal $190.

(b) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(4) Annual license renewal fee for a retired license; $15

(5) Change of status to Active to Inactive

(a) Licensure change to inactive status $190

(b) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(6) Inactive license to active;

(a) Licensure change to active status $400

(b) Annual Prescription Drug Monitoring Program (PDMP) fee $35

(c) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(7) Inactive or active license to a retired license; $15.

(8) Lapsed license to Active License

(a) License Restoration fee $150

(b) Licensure change to active status $400

(c) Annual Prescription Drug Monitoring Program (PDMP) fee $35

(d) Annual fee Oregon Workforce Database for active and inactive licenses; $2;

(9) Natural childbirth initial certification and renewal; $60.

(10) Miscellaneous

(a) Oregon Jurisprudence Examination; $200

(b) Licensure Application Processing Fee $50

(c) Criminal record check; $50

(d) Wall certificate; $25;

(e) Mailing list of licensees; $50;

(11) All Board fees and fines are non-refundable.

(12) The remittance of any application fee, license fee, yearly renewal fee, or fee otherwise imposed by the Board shall be made via credit or debit card.
