Section 809-015-0010 - Nonrestoration(1) After five consecutive years of non-renewal, a registration can no longer be restored. (2) A person past the 5 year restoration window must re-apply as a new applicant, meeting all current Board requirements for registration. This includes but is not limited to passage of national and specialty examination required by the Board unless previously passed as verified by the Board. (3) Upon application to the Board for a new registration, a previous registration number shall not be reinstated. Or. Admin. Code § 809-015-0010
