Section 411-325-0090 - Change of Ownership, Legal Entity, Legal Status, Management Corporation(1) The service provider must notify the Department in writing of any pending change in ownership or legal entity, legal status, or management corporation. (2) A new license is required upon change in ownership, legal entity, or legal status. The service provider must submit a license application and required fee at least 30 days prior to change in ownership, legal entity, or legal status.Or. Admin. Code § 411-325-0090
