Section 410-133-0160 - Licensed Practitioner Recommendation

Requests for reimbursement for Medicaid-covered SBHS pursuant to a Medicaid-enrolled child or young adult's IPOC must be supported by a current, within twelve (12) months of the date-of-service, written recommendation from a licensed health care practitioner.
