Section 333-076-0114 - ASC Complaints(1) Any person may make a complaint verbally or in writing to the Authority regarding an allegation against an ASC of a violation of health care facility licensing laws or regulations.(2) The identity of a person making a complaint and any personally identifiable information, as that is defined in ORS 432.005, is confidential and not subject to disclosure under ORS 192.311 to 192.478.(3) An investigation will be carried out as soon as practicable after receipt of a complaint in accordance with OAR 333-076-0116.(4) An ASC shall post a notice in the facility, in a prominent place and size that must include, but is not limited to the following: "If you have concerns about this ambulatory surgical center and the services provided here, contact the Oregon Health Authority, Health Care Regulation and Quality Improvement Program: 800 NE Oregon Street, Suite 465, Portland OR 97232; 971-673-0540."
