Section 309-022-0210 - Issuance of a License(1) The Division shall issue a license within 30 days of the submission of a completed application. The license shall state the name of the licensee, the provider and expiration date. (2) The license shall be placed in the licensee's personnel file and be easily visible. (3) An initial license is valid from the time of issuance until the expiration date, which will be September 30th of the following calendar year. Or. Admin. Code § 309-022-0210
