Section 670:20-39-3 - Affidavit of body change

If a vehicle is converted to another type, such as adding a fifth (5th) wheel to a truck to use as a truck tractor, a Body Change affidavit (SOK Form 701-9) must be completed. A title receipt showing the new designation and reregistration will be issued accordingly.
