Section 428:10-3-1 - Request format

(a) State governmental entities requesting completion of existing projects or new capital projects must prepare and submit a decision package as provided by the Commission and the Office of Management and Enterprise Services.

(b) The decision package shall include, at a minimum, the following information:

(1) Description of project;

(2) Length and phasing of project;

(3) Estimate of project costs (by fiscal year);

(4) Sources of funds to be used for the project;

(5) Operational cost changes; and

(6) Supporting documentation to justify project need.

(c) All project submissions shall be rated using the commission's Capital Outlay Request Evaluation Criteria by the agency submitting the request. The Commission's Capital Outlay Request Evaluation Criteria are available by request through the Office of Management and Enterprise Services.

(d) Project requests that do not contain information required by (b) and (c) of this section shall not be considered for inclusion in the Capital Improvements Plan or Annual Capital Budget.
