Section 310:679-10-11 - [Effective until 9/14/2024] Documentation requirements
(a) When submitting documentation for a bachelor's or associate degree, an official copy of the transcript(s) should be sent to the Department.
(b) The applicant shall submit a letter signed by a licensed long-term care administrator, medical director, director of nurses, or registered nurse on company letterhead attesting to the applicant's work experience.
