Section 310:641-3-14 - Severance of action, amendment, and re-instatement(a) The issuance or renewal of a license after notice of a violation(s) has/have been given, shall not constitute a waiver by the Department of its power to rely on the violation(s) for subsequent license revocation or other enforcement action which may arise out of the notice of the violation(s).(b) Any change in the name of the service, level, service area, addition of substation, or type of service shall necessitate an application to amend the license and shall be accompanied by a fee of one hundred dollars ($100.00).(c) Addition of a substation that expands the service area shall comply with 310:641-3-11.(d) Changing or moving the location of a substation requires written notification to the Department.(e) If an existing license is placed on probation or suspension, a fee of one hundred ($100.00) dollars, in addition to any other provision of the action, shall be submitted prior to re-instatement of the license to full privilegeOkla.
