Section 310:395-7-1 - Purpose(a) This Subchapter ensures that all applicants meet the requirements specified in the Act.(b) Unless otherwise indicated, an applicant shall submit all required information and documentation of credentials on official Department forms and in the manner prescribed by the Department.
