The applicant for a barber school license shall address a letter to the Oklahoma State Department of Health, Barber Division, 1000 Northeast 10th Street, Oklahoma City, Oklahoma, 73117-1299, requesting the necessary application blanks. The applicant shall state in the communication, the name and address of the proposed owners. If a corporation, the names and addresses of the officers and principal stock owners shall be listed.
