Section 252:307-3-2 - Contact information

In addition to other information required by this Chapter, an application shall contain the following information:

(1) The name, mailing address, street address, telephone number, e-mail address and telefax number (if any) of the applicant.
(2) The signature, typewritten name, address, telephone number and telefax number (if any) of the owner or owner's authorized representative.
(3) The name, mailing address, street address, telephone and telefax number (if any) of the applicant laboratory's authorized manager.
(4) The location(s) (address or legal description) of the laboratory, including county and driving directions and latitude/longitude.
(5) Identification of the accreditation class and categories, analytes and/or methods sought.
