Section 150:1-13-1 - Requests; petition contents
(a) All requests for declaratory rulings as to the applicability of any rule or order of the Department shall be made by filing a petition with the Department requesting such a ruling.

(b) The petition shall identify the rule or order questioned, the date on which rule or order became effective, and shall set forth the contents of the rule or order.
(c) The petition shall include a brief statement of the issue or issues raised by the rule or order which caused such a request to be made, and a statement as to the petitioner's personal interest in the ruling of the Department and how much a ruling by the Department will affect those interests.
(d) The petition shall set forth names known to petitioner of persons or entities who may have an interest in the outcome. The Department's hearing examiner may entertain oral presentation or argument on the request of petitioner, and the Department may, on its own motion, request argument, briefing or presentation from the petitioner and from Department personnel.
