Section 901:10-4-02 - General permit to operate coverage(A) A general permit to operate as described in rule 901:10-4-01 of the Administrative Code is issued by the director for a category of facilities located at multiple sites. (B) Each person eligible for a general permit to operate shall follow the requirements in rule 901:10-4-03 of the Administrative Code. (C) Upon issuance of a general permit to operate and issuance of an individual certificate of coverage, the individual permit to operate shall be terminated. All previous permits issued to a facility that can be covered by a general permit to operate are revoked upon request of the owner or operator, termination of the individual permit and issuance of the certification of coverage. (D) Any person covered under a general permit to operate may choose to pursue an individual permit for any facility covered by this rule. Ohio Admin. Code 901:10-4-02
