Section 3770:1-9-326 - Game rule number three hundred twenty-six
(A) Title and term. Ohio lottery commission game rule number three hundred twenty-six, "Win for Life," shall be conducted at such times and for such periods as the commission may determine. For the purpose of this rule, "sales cycle" shall mean any such period, including reprints, beginning on the date when ticket sales are commenced and continuing through the date established by the director as the date on which agents are to make their final settlement with respect to tickets allocated to them during the period in game rule number three hundred twenty-six.
(B) General design.
(1) Game rule number three hundred twenty-six is an instant lottery game within which are contained three separate games.
(2) Holders of valid tickets will remove the covering off the entire main play area to reveal five "Lucky Numbers," ten "Your Numbers" and ten corresponding prize values. If the player matches any one or more of "Your Number(s)" to one or more of the "Lucky Numbers(s)," the player wins the corresponding prize value shown below the matching "Your Numbers." If the player matches any one of "Your Number(s)" to any one of the "Lucky Numbers," and the "Life" play symbol appears below the matching "Your Number," the player wins the "Life Prize" which is one thousand dollars a week for life (paid out at fifty-two thousand dollars a year before taxes until a minimum of one million dollars is reached, or for the life span of the original prize winner, whichever is greater). If a player matches any one of "Your Numbers" to one of the "Lucky Numbers," and the corresponding prize value revealed is a "TPD" symbol, the player wins one thousand dollars automatically, and, upon the filing of a valid claim at an Ohio lottery commission office, shall also automatically be entered in the Top Prize Drawing ("TPD") to win the "Life Prize." The Top Prize Drawing shall be conducted in accordance with Ohio lottery rules and procedures established and approved by the director. The duration of the "Life Prize" shall be determined by the original prize winner's life span. If there is more than one prize winner of the "Life Prize," the duration of the prize winnings shall be determined by one designated prize winner's life span. Subject to provisions of rule 3770:1-8-01(B) (6)(c), should the life span of the originally designated winner be less than twenty years, then the prize winner's beneficiary(ies) shall be entitled to continue receiving annual payments of fifty-two thousand dollars a year before taxes until one million dollars is reached. Should the "Life Prize" be claimed by any "person" as defined in rule 3770:1-1-01(K)(2) of the Administrative Code other than an individual or a group of individuals (specifically; a corporation, business trust, estate, trust, partnership or association), the maximum amount such person shall be entitled to shall be the minimum "Life Prize" amount of one thousand dollars a week for life paid out at fifty-two thousand dollars a year before taxes until one million dollars is reached.

Also, the player will remove the covering over two additional play areas entitled "Quick $50" and "Quick $100." If the player reveals a "Star" play symbol in the "Quick $50" play area, the player wins fifty dollars automatically. If the player reveals a "Moneybag" play symbol in the "Quick $100" play area, the player win one hundred dollars automatically. There are twelve chances to win one each ticket, and a player may win up to twelve times on one ticket.

(3) As used in this rule, "prize award" shall mean one of the following monetary figures which is the total of all winning prize values appearing on the ticket: five dollars, ten dollars, twenty dollars, thirty dollars, fifty dollars, one hundred dollars, two hundred dollars, five hundred dollars, one thousand dollars, ten thousand dollars and one million dollars (which is the minimum "Life Prize" as described in Section (B)(2) of this Rule). Each ticket in game rule number three hundred twenty-six shall be imprinted in such a way that prize awards from the set listed above may be won.
(C) Price of tickets. The price of a ticket issued by the commission in game rule number three hundred twenty-six shall be five dollars.
(D) Structure, nature and value of prize awards.
(1) There shall be two types of prizes in game rule number three hundred twenty-six, a "regular prize award" and a "TPD" prize award.
(2) The only "prize values" which shall appear on a ticket in game rule number three hundred twenty-six are those listed as follows: one dollar, two dollars, three dollars, five dollars, ten dollars, twenty dollars, thirty dollars, fifty dollars, one hundred dollars, two hundred dollars, five hundred dollars, the "TPD" symbol corresponding to a prize value of one thousand dollars, five thousand dollars, ten thousand dollars and a "Life Prize" as described in Section (B)(2) of this Rule. Prize values and play symbols appearing on any ticket in game rule number three hundred twenty-six shall be concealed by an opaque covering which may be scratched off by the holder of the ticket to reveal the underlying prize values and play symbols.
(a) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by five of "Your Numbers" for one dollar five times shall win a regular prize award of five dollars;
(b) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by three of "Your Numbers" for one dollar once and two dollars two times shall win a regular prize award of five dollars;
(c) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by two of "Your Numbers" for two dollars once and three dollars once shall win a regular prize award of five dollars;
(d) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for five dollars once shall win a regular prize award of five dollars;
(e) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by five of "Your Numbers" for two dollars five times shall win a regular prize award of ten dollars;
(f) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for one dollar ten times shall win a regular prize award of ten dollars;
(g) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by two of "Your Numbers" for five dollars two times shall win a regular prize award of ten dollars;
(h) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for ten dollars once shall win a regular prize award of ten dollars;
(i) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for two dollars ten times shall win a regular prize award of twenty dollars;
(j) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by four of "Your Numbers" for five dollars four times shall win a regular prize award of twenty dollars;
(k) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by seven of "Your Numbers" for two dollars five times and five dollars two times shall win a regular prize award of twenty dollars;
(l) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for twenty dollars once shall win a regular prize award of twenty dollars;
(m) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by three of "Your Numbers" for ten dollars three times shall win a regular prize award of thirty dollars;
(n) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by six of "Your Numbers" for five dollars six times shall win a regular prize award of thirty dollars;
(o) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for three dollars ten times shall win a regular prize award of thirty dollars;
(p) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by eight of "Your Numbers" for one dollar once, two dollars two times and five dollars five times shall win a regular prize award of thirty dollars;
(q) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for thirty dollars once shall win a regular prize award of thirty dollars;
(r) Holder of a valid winning ticket on which a "Star" symbol is revealed in the "Quick $50" play area for fifty dollars shall win a regular prize award of fifty dollars;
(s) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for five dollars ten times shall win a regular prize award of fifty dollars;
(t) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for two dollars five times, five dollars two times and ten dollars three times shall win a regular prize award of fifty dollars;
(u) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for fifty dollars once shall win a regular prize award of fifty dollars;
(v) Holder of a valid winning ticket on which a "Moneybag" symbol is revealed in the "Quick $100" play area for one hundred dollars shall win a regular prize award of one hundred dollars;
(w) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for fifty dollars once; and on which a "Star" symbol is revealed in the "Quick $50" play area for fifty dollars shall win a regular prize award of one hundred dollars;
(x) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for ten dollars ten times shall win a regular prize award of one hundred dollars;
(y) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for one hundred dollars once shall win a regular prize award of one hundred dollars;
(z) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for twenty dollars ten times shall win a regular prize award of two hundred dollars;
(aa) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for five dollars ten times; on which a "Star" symbol is revealed in the "Quick $50" play area for fifty dollars; and on which a "Moneybag" symbol is revealed in the "Quick $100" play area for one hundred dollars shall win a regular prize award of two hundred dollars;
(bb) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for two hundred dollars once shall win a regular prize award of two hundred dollars;
(cc) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for ten dollars once, thirty dollars three times and fifty dollars six times; and on which a "Moneybag" symbol is revealed in the "Quick $100" play area for one hundred dollars shall win a regular prize award of five hundred dollars;
(dd) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for twenty dollars five times and fifty dollars five times; on which a "Star" symbol is revealed in the "Quick $50" play area for fifty dollars; and on which a "Moneybag" symbol is revealed in the "Quick $100" play area for one hundred dollars shall win a regular prize award of five hundred dollars;
(ee) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for five hundred dollars once shall win a regular prize award of five hundred dollars;
(ff) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" and a "TPD" prize symbol is revealed for one thousand dollars shall win a regular prize award of one thousand dollars, and, upon the filing of a valid claim at an Ohio lottery commission office, automatic entry into the Top Prize Drawing to win the "Life Prize" as described in Section (B)(2) of this Rule;
(gg) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by all ten of "Your Numbers" for five hundred dollars ten times shall win a regular prize award of five thousand dollars;
(hh) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for five thousand dollars once shall win a regular prize award of five thousand dollars;
(ii) Holder of a valid winning ticket on which one or more of the "Lucky Numbers" are matched by two of "Your Numbers" for five thousand dollars two times shall win a regular prize award of ten thousand dollars;
(jj) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" for ten thousand dollars once shall win a regular prize award of ten thousand dollars; and
(kk) Holder of a valid winning ticket on which one of the "Lucky Numbers" is matched by one of "Your Numbers" and the corresponding prize value revealed is the "Life" symbol, shall win a regular prize award of a "Life Prize" as described in Section (B)(2) of this Rule.
(E) Number of prize awards. The number of prize awards in any sales cycle of game rule number three hundred twenty-six will depend upon the number of tickets sold during that cycle. However, distribution and availability for public purchase may be affected by, but not limited to, circumstances such as stolen, defective, missing, damaged, promotional or returned tickets. Tickets shall be printed in accordance with this rule using random techniques in order that:
(1) Combinations winning each prize award are randomly distributed throughout all tickets printed in any given ticket issuance; and
(2) Mathematical reasoning indicates that the number of winning tickets sold per four million tickets sold should be as follows:
(a) One ticket winning the "Life Prize" as described in Section (B)(2) of this Rule;
(b) Two tickets winning ten thousand dollars once to win ten thousand dollars;
(c) Two tickets winning five thousand dollars two times to win ten thousand dollars;
(d) Four tickets winning five thousand dollars once to win five thousand dollars;
(e) Six tickets winning five hundred dollars ten times to win five thousand dollars;
(f) Twenty tickets winning five hundred dollars once to win five hundred dollars;
(g) Twenty tickets winning twenty dollars five times and fifty dollars five times, fifty dollars in the "Quick $50" play area, and one hundred dollars in the "Quick $100" play area to win five hundred dollars;
(h) Twenty tickets winning ten dollars once, thirty dollars three times and fifty dollars six times, and one hundred dollars in the "Quick $100" play area to win five hundred dollars;
(i) Twenty tickets winning two hundred dollars once to win two hundred dollars;
(j) Twenty tickets winning five dollars ten times, fifty dollars in the "Quick $50" play area, and one hundred dollars in the "Quick $100" play area to win two hundred dollars; and
(k) Twenty tickets winning twenty dollars ten times to win two hundred dollars.
(3) Mathematical reasoning indicates that over a sufficiently large number of tickets sold, the average number of winning tickets per two hundred thousand tickets sold should be as follows:
(a) One ticket revealing a "TPD" prize symbol to win one thousand dollars, and, upon the filing of a valid claim at an Ohio lottery commission office, automatic entry into the Top Prize Drawing to win a "Life Prize" as described in Section (B)(2) of this Rule;
(b) Twenty-three tickets winning one hundred dollars once to win one hundred dollars;
(c) Fifty tickets winning ten dollars ten times to win one hundred dollars;
(d) Forty-one tickets winning fifty dollars once, and fifty dollars in the "Quick $50" play area to win one hundred dollars;
(e) Two hundred tickets winning one hundred dollars in the "Quick $100" play area to win one hundred dollars;
(f) Six hundred tickets winning fifty dollars once to win fifty dollars;
(g) Seven hundred tickets winning two dollars five times, five dollars two times and ten dollars three times to win fifty dollars;
(h) Seven hundred tickets winning five dollars ten times to win fifty dollars;
(i) Two thousand tickets winning fifty dollars in the "Quick $50" play area to win fifty dollars;
(j) One hundred tickets winning thirty dollars once to win thirty dollars;
(k) Five hundred tickets winning one dollar once, two dollars two times and five dollars five times to win thirty dollars;
(l) Two hundred tickets winning three dollars ten times to win thirty dollars;
(m) Three hundred tickets winning five dollars six times to win thirty dollars;
(n) Three hundred tickets winning ten dollars three times to win thirty dollars;
(o) One thousand tickets winning twenty dollars once to win twenty dollars;
(p) One thousand tickets winning two dollars five times and five dollars two times to win twenty dollars;
(q) One thousand tickets winning five dollars four times to win twenty dollars;
(r) One thousand tickets winning two dollars ten times to win twenty dollars;
(s) Two thousand tickets winning ten dollars once to win ten dollars;
(t) Two thousand tickets winning five dollars two times to win ten dollars;
(u) Four thousand tickets winning one dollar ten times to win ten dollars;
(v) Four thousand tickets winning two dollars five times to win ten dollars;
(w) Four thousand tickets winning five dollars once to win five dollars;
(x) Four thousand tickets winning two dollars once and three dollars once to win five dollars;
(y) Eight thousand tickets winning one dollar once and two dollars two times to win five dollars; and
(z) Eight thousand tickets winning one dollar five times to win five dollars.
(F) Tickets sold. An instant ticket game may continue to be sold even when some or all of the top prizes have been claimed.
(G) Frequency of prize drawings.
(1) Random imprinting of prize awards on all tickets issued in game rule number three hundred twenty-six shall be accomplished in a manner which complies with the commission's rules and procedures.
(2) When a ticket issued in game rule number three hundred twenty-six is sold or deemed sold in accordance with this rule and the covering material over any of the prize values and play symbols has been removed, the holder shall be deemed to have drawn the prize values and play symbols on that ticket which determine whether the holder is entitled to a regular prize award. All regular prize awards shall be deemed announced no later than the last day of the sales cycle of game rule number three hundred twenty-six in which the ticket was sold.
(3) The Top Prize Drawing to award one grand prize of a "Life Prize" as described in Section (B)(2) of this Rule shall take place in accordance with rules and procedures established and approved by the director, and specifically, rule 3770:1-6-05

of the Administrative Code governing "Prize drawings" and rule 3770:1-6-01

of the Administrative Code governing "Special lottery drawings." Such drawings shall be held on dates determined and announced by the director, however, the earliest Top Prize Drawing date shall be one hundred eighty days from the game close date or when all tickets winning a prize value corresponding with a "TPD" symbol have been claimed, whichever is later.

(H) Special claim, entry, receipt and validation procedures. The director shall establish special claim, entry, receipt and validation procedures, including procedures for validation by agents of tickets winning prize awards which are to be paid by agents in accordance with commission rules, as well as procedures for entry into the Top Prize Drawings described in Section (G)(3) of this Rule. Prize awards shall be claimed within the time limits set forth by commission rules.
(I) Validity of tickets.
(1) A mechanical error in printing prize awards, symbols, words or other numbers on a ticket shall not automatically invalidate that ticket. To the extent feasible, the director shall establish procedures by which the holder of any ticket on which information is incorrectly printed due to mechanical malfunction may be advised of correct information for the ticket. If it is not technically feasible to recover the information from a mechanically misprinted ticket, the director may declare the ticket void and the holder shall be entitled to a return of the ticket price or a replacement ticket of comparable price.
(2) In addition to, but not in limitation of, all other power and authority conferred on the director by the commission's rules, the director may declare a ticket in game rule number three hundred twenty-six void if it is stolen, unissued, deactivated, not sold or deemed not sold in accordance with commission rules; if it is illegible, mutilated, altered, counterfeit, misregistered, reconstituted, miscut, defective, printed or produced in error or incomplete; or if the ticket fails any of the validation tests or procedures established by the director. The commission's liability and responsibility for a ticket declared void, if any, is limited to refund of the retail sales price of the ticket or issuance of a replacement ticket of comparable price.
(J) Director's conduct of game rule number three hundred twenty-six.
(1) The director shall conduct game rule number three hundred twenty-six in a manner consistent with the Lottery Act, the rules of the commission, including without limitation, this rule and the regulations of the director. As deemed necessary or advisable, the director shall adopt management regulations, orders or directives to implement and operate this lottery game. The director shall inform the public of the provisions of this rule and the procedures established pursuant hereto which affect the play of game rule number three hundred twenty-six. The director reserves the right to declare any instant game as unavailable for sale at any time to maintain game integrity and accountability.
(2) Names and definitions of elements of game rule number three hundred twenty-six used in this rule are to be considered generic terms used solely for purposes of this rule. In actual operation, game rule number three hundred twenty-six and these elements may be given names or titles chosen by the commission.
