For a bingo session, raffle drawing, sports-pool board, calcutta board, paddlewheel excluding the use of a table, and twenty-one or poker tournament, an employee shall legibly print this information on a prize register or similar document when a prize is issued to a player:
2. Game type: a. Bingo - Date of the session and game number.b. Raffles - Date of the drawing, winning ticket number, gaming stamp number (if applicable), and initials of two employees who conducted the drawing unless the initials are on another document.c. Sports pools - Date of the sports event, winning score, and gaming stamp number.d. Twenty-one or poker tournament - Date of the tournament.e. Calcutta - Date of the sports event and gaming stamp number.f. Paddlewheel excluding the use of a table - Date of the event, card number, winning ticket number, and gaming stamp number;3. Amount of a cash prize or a description, cost, and retail price of a merchandise prize;4. Full name of player and for a bingo prize exceeding one hundred dollars, signature of player;5.
