Section 87-04-02-01 - Companion animal recordsPatient records must contain the following information in electronic or written legible form:
1. Client name, address, and phone numbers.2. Patient name, breed, description, sex, and age.3. Results of any patient physical examinations, diagnostic and treatment plans, and results of diagnostic and treatment procedures.4. Any medications used, including route of administration and dosage.5. Evidence of the client's consent for all surgical procedures and euthanasias. Although it is preferred that a signed consent form be obtained from each client, such is not required as long as the records reflect oral consent was received.6.
