In accordance with requirements of North Dakota Century Code section 14-02.1-07, the state department of health and consolidated laboratories has a Report of Induced Abortion Form (Appendix B) to be executed in duplicate. The form shall be used by the hospital or facility in which an induced abortion is performed.
