(a) A fee of three hundred and fifty dollars ($350.00) is due at the time of application for a perfusion license and a fee of one hundred and seventy five dollars ($175.00) is due at the time of application for a provisional perfusion license. No portion of the application fee is refundable.(b) A fee of three hundred and fifty dollars ($350.00) shall be paid to the North Carolina Medical Board for biennial renewal of a perfusion license and a fee of one hundred and seventy five dollars ($175.00) for annual renewal of a provisional perfusion license.(c) A late fee of one hundred dollars ($100.00) shall be charged to those who fail to renew timely a perfusion license or a provisional perfusion license.21 N.C.
