Form BC-2 and accompanying schedules 1 to 7 must be executed and verified. Where a license has expired (see section 5812.7 of this Subchapter) and an application for another license is made, form BC-2 shall be used together with schedules 1 to 7. However, where a license is to be amended and there is no change in the contents of any particular schedule from the schedules previously filed, a statement to that effect shall appear in the application and those schedules may be omitted.
